Information you provide when you contact Dr. K Learning Studio or request an introductory assessment may include your name, email, phone number, student grade or course, academic needs, preferred session format, and availability. For ongoing tutoring requests through Wix Bookings, the form collects your first and last name, email address, and required phone number. For a session at the student’s location, it also collects the session address. You may add an optional message. Your request also includes your selected appointment date and time.

 

We use this information to respond to inquiries, review and manage appointment requests, and prepare for assessments. Please share only information relevant to tutoring. Do not submit passwords, payment details, government identification, or sensitive medical records.

Parents and guardians

For students under 18, a parent or guardian should complete the inquiry or assessment form using the adult’s contact details.

Website and forms

 

This website is hosted by Wix and uses Wix Bookings for ongoing tutoring requests. The assessment form is hosted by Google Forms, which processes information under Google’s privacy policy. For information about how Wix processes personal information, see Wix’s privacy policy.
